Records requests must be made in person, Monday thru Friday between the hours of 8 a.m. and 5 p.m. You can also make request via mail. Payment must be included if you chose to mail in your form.
To submit a Recording Records Request:

- Fill in the information on the form
- Print and sign the form
- Deliver your Recording Records Request Form and required deposit to the address listed below
- Payment must be made via cash or check
